Request a Permit

A permit is often required to enter a confined space. The Safety Assessment form indicates whether or not a permit is required. You must have permission to view confined space files to request a permit.

To request a permit:

  1. Select Confined Space > Request a Permit.
  2. In the Confined Space section, select the Site, Area, Equipment, and Confined Space.

    Tip: You can also access this form from the Find/Create Files tab. Find an existing confined space assessment, move the pointer over it, and select Request a Permit. When opened in this fashion, you do not need to fill in the Confined Space section because the software already has this information.

  3. In the Permit Information section fill in the Date of Entry, Start Time, End Time, Purpose of Entry, and, if desired, Entry Supervisor, and Return Completed Form To. If you leave the Entry Supervisor field blank, the permit will be automatically granted.
    • Entry Supervisor: The entry supervisor is the person who must approve the permit. This list contains users who have the rights to view confined space documents.
    • Return Completed Form To: This list also contains users who have the rights to view confined space documents. The person specified here is able to close the permit. If left blank, then the person requesting the permit must close it.
  4. (Optional) Click Add Entrants to specify the people who will be entering the confined space. A form opens with a list of people trained to enter confined spaces. (This is specified in Setup Users.) Select the check box next to each person who needs to enter this confined space. Click OK.
  5. (Optional) Click Add Attendants to specify people who will wait outside the confined space. A form opens with a list of people trained to enter confined spaces. (This is specified in Setup Users.) Select the check box next to each attendant. Click OK.
  6. Click Request Permit. The permit is sent to the Entry Supervisor for approval if one has been specified; if not, the permit is automatically granted. To check on the status see Open Confined Space Requests (Permits).
